Parish Registration/Update

The registration form may be used to submit a NEW registration or UPDATE an existing registration.

There are two ways to complete the form: 

Option 1:  Please print both pages, Parish Registration page 1 and  Parish Registration page 2 , fill it out, scan and email it to us at office@stjudelewes.org .  You may also mail it to the office:  St Jude Catholic Parish, 152 Tulip Drive, Lewes, DE  19958-1689.

Option 2:  If you would like to complete the form online, please fill in the form below.  

DON'T FORGET TO CLICK ON "SUBMIT" AT THE BOTTOM OF THE PAGE!  The online registration form DOES NOT ALWAYS WORK properly using a mobile device (iPad or other tablet device, or iPhone, Android, or other smart phones).
